Hybrid/Remote Learning Opt-In Form

1. Login to Skyward Family Access (Online Forms are not available in Skyward Moblie App)

   • Support: If you do not have a username and password for Family Access, please contact [e-mail].
     
2. From the Family Access Home Screen, click Online Forms.
   • A window will appear for each child you have in the district (You will have to complete a form for every child).
   •  Click ‘Fill Out Form’ for the child you would like to complete the form.
     
3. Once the form page opens, you will then see a dropdown to choose between Hybrid Learning and Remote Learning. Choosing "Hybrid learning" allows your student to participate in on-site classes when we make the switch to our hybrid learning. Choosing "Remote learning" commits your student to continuing with remote learning only throughout the spring semester.
   
4. Once you've decided, click the "Complete Step 1 and move to Step 2" button located at the bottom of the form. This will take you to the final confirmation screen to review your selection. Click the "Submit Hybrid/Remote Opt-In" form to complete this process.
